Proxy Account Access for family and caregivers
What is Proxy Access?
Proxy Access is a MyChart feature that allows family members and caregivers to securely access another person’s MyChart account in order to view health records and communicate with providers on behalf of the person under their care via their own MyChart account.
Who is eligible to use Proxy Access?
- Parents and guardians of children under 18 years of age. Please note:
- For children under 11, you will be granted full access to your child’s MyChart account.
- For children 12 -17 years, you will be granted limited access to your child’s Montefiore MyChart account and will be able to request an appointment but not view medical information.
- Caregivers of adults over 18 years of age.
How do I request Proxy Access to an account for a family member or adult with diminished capacity?
- Log in to your own MyChart account. If you do not have your own account, sign up on the log in page. (Please note: This feature is available for adults over 18 only.)
- Navigate to the Family & Caregiver Proxy Access page in the menu.
- Select the option for “Request Access”
- Fill out the corresponding form for children under 18 or adults with diminished capacity to request access.
- After your request is approved, you will be able to toggle between your own account and proxy accounts by selecting the corresponding account in the top righthand menu.
How do I invite a family member or caregiver to receive Proxy Access to my account?
- Log in to your own MyChart account (If you do not have your own account, sign up on the log in page.)
- Navigate to the Family & Caregiver Proxy Access page in the menu.
- Select the option for “Invite Access”
- Fill out the invitation form.
- After your request is approved, you will be able to toggle between your own account and proxy accounts by selecting the corresponding account in the top righthand menu.
