If you are a patient aged 14-17, please contact your doctor’s office for assistance in signing up for Montefiore MyChart. If you do not have a doctor at Montefiore, you can call 1-800-MD-MONTE (800-636-6683) for help finding a new doctor.
If you are an adult seeking access to Montefiore MyChart on behalf of a patient age 17 or younger, you must be listed as the guarantor and/or emergency contact for the patient. Please first log-in or sign up for your own MyChart account. You may then request access via Share My Record / Friends and Family Access.
